ID Card section:
To ensure enrollment in the Defense Enrollment Eligibility Reporting System (DEERS), provide Common Access Cards (CAC) and other Official Identification Cards to Soldiers, Family Members, retirees, civilians, and contractors in New Jersey, Pennsylvania and Southern New York.
The ID Card Section is a walk-in facility only, no appointments.
Sponsor must be present when a Family Member is renewing, and/or replacing a Family Member ID card. Exceptions to this rule are:
The Family Member has an unexpired General Power of Attorney and or has a completed DD Form 1172-2 generated by a military ID work center.
Acceptable Forms of Primary Photo ID:
Military ID, Driver's license, Passport, State, Federal or Local government ID card (Badges are not accepted). All identification documents must be originals or court certified copies. There are no exceptions.
Acceptable Forms of Secondary ID:
Social Security card, Birth certificate (must have a file number (if not original must be court notarized copy)), Voter's Registration Card, School ID with photo, Permanent Resident Card, INS Form 1-151 or 1-551, and or Foreign National ID with picture.

Most common and used documents are:
Two forms of valid/picture ID, Extension order (if applicable), Reenlistment documents, Promotion order, DD Form 214, Social Security Card, Original/court certified copy of marriage certificate with seal, Sponsor's divorce decree (if applicable).
